Just removed the lastpass extension and can confirm that it fixed the issue. Gregory King it looks like the form has an issue with password managers.

George Panagopoulos: We've had a report previously that Bitwarden (which manipulates the page) causes issues that we haven't yet been able to resolve. Be useful to know what your password manager is.
